Stygian: Outer Gods is a first-person survival horror and role-playing game set within the grim, cosmic-horror universe of Stygian: Reign of the Old Ones. Serving as a narrative prequel, the story takes place just before the catastrophic event known as the Black Day. Players step into the shoes of Jack, an investigator lured to the isolated, fog-shrouded coastal town of Kingsport under the pretense of finding his presumed-dead father.

The gameplay shifts the series away from its original top-down CRPG perspective into an immersive first-person survival-horror experience. Exploration takes place across a hostile, maze-like environment of derelict buildings, snowy settlements, dark mines, and lighthouses. Survival demands careful resource management, environmental puzzle-solving, crafting tools like lockpicks and bandages, and utilizing a distinct tarot-card progression system that grants both major “seer” abilities and minor skill boosts.

Combat is hazardous and heavily punishing, turning single enemies into lethal threats that require stealth, glass-bottle distractions, and careful crouching rather than direct confrontation. Players must also actively manage a fragile sanity meter, which erodes near supernatural anomalies, dark visions, and terrifying entities tied to the ancient Outer Gods. Your moral and character choices—shaped initially at a bathroom mirror and expanded through side quests—directly influence the narrative’s path, encounters, and multiple possible conclusions.
